Anti-Wrinkle Injections vs Dermal Fillers: What’s Actually the Difference?

It’s one of the most common questions we hear in consultations — and one of the most commonly confused pairings in aesthetics. Anti-wrinkle injections and dermal fillers are often mentioned in the same breath, sometimes even used interchangeably, but they do genuinely different jobs. Understanding the difference matters, because using the wrong one for the wrong concern is how people end up unhappy with results that were never going to solve the problem in the first place.

What anti-wrinkle injections actually do

Anti-wrinkle injections work by temporarily relaxing the small muscles responsible for certain facial expressions. Lines that form from repeated movement — a furrowed brow, crow’s feet at the corners of the eyes, horizontal forehead lines — soften because the underlying muscle activity that creates them is reduced. The skin itself isn’t altered structurally; the treatment addresses the movement that causes the line to form.

This is why anti-wrinkle injections are typically most effective on “dynamic” lines — the ones that appear when you frown, squint, or raise your eyebrows — rather than lines that are already etched into resting skin.

What dermal fillers actually do

Dermal fillers work on a completely different principle. Rather than affecting muscle movement, they add volume and structure — most commonly using hyaluronic acid, a substance that occurs naturally in the skin. Fillers are used to restore volume lost to ageing, add subtle structure and definition, or soften static lines and hollows that are present even when the face is at rest — think nasolabial folds, thinning lips, or hollowing under the eyes.

So which one do you need?

Often, honestly, it’s not a choice between the two — it’s about matching the right tool to the right concern:

  • Expression lines that only show when you move your face (frown lines, crow’s feet) → typically anti-wrinkle injections
  • Volume loss, hollowing, or lines present at rest (cheeks, under-eyes, nasolabial folds, lips) → typically dermal filler
  • A combination of both concerns → often a considered mix of the two, planned around your specific face and goals

This is exactly why a proper consultation matters more than a treatment menu. The honest answer to “which one do I need” is almost always “it depends” — on your anatomy, your concerns, and what’s actually driving the change you’re seeing in the mirror. At Clinic Platina, that conversation always comes before any recommendation, not after.

A note on approach

We don’t believe in treating either of these as a default. The goal isn’t simply to reduce a line or add volume for its own sake — it’s to make a considered, individual decision about what will actually help your skin and face look like a well-rested, more at-ease version of yourself. That’s the whole basis of how we approach every consultation: safety, individuality, and honesty, in that order.

If you’re not sure which approach — if either — is right for you, that’s precisely what a consultation is for.
